Description

Provides for the expansion of primary healthcare services for patients in the area of clinical specialization. Clinic experience preferred.

Licensed or eligible for licensing in the State of WI as an RN. Holds a Master of Science Degree in Nursing with Family Nurse Practitioner emphasis. Certified or eligible for certification in the State of WI as a APNP.


OR


Certification from the National Commission on Certification of Physician Assistants. CPR certified or obtained within first 90 days of employment. Current State of WI PA License. DEA Certification required. Graduate of an accredited Physician Assistant program.

This position will be 20 hours per week with the following schedule:

  • 12 hours per week: Grande - Rubicon 8:00am-11:00am and Thursday 1:00pm-4:30pm, as well as additional coverage for Grande CLinic in Fond du Lac & Brownsville.
  • 8 hours per week: Waupun Work & Wellness 7:00am-3:00pm one day per week.
