SSM Health Agnesian Health Hospice team is searching for a full time (.8 ) Nurse Practitioner to our hospice team. This provider will support our hospice team by meeting with patients in their home, our Fond du lac Hospice Facility, Green Lake Hospice facility and Fond du Lac At Home Program. These visits assess appropriateness of care based on symptoms and diagnosis. Primary coverage area for in-home visits are centered in the greater Fond du lac area. Participation in bi-weekly interdisciplinary care plan meetings will be part of this role. Ideal candidate has experience practicing in palliative care or hospice environment.

  • Monday through Friday
  • In-home and hospice facility patient assessments for new and 3rd benefit period hospice needs
  • Focus on symptom and medication management
  • Educate staff with a variety of topics
  • Be a part of a collaborative team and assist as needed
  • Responsible to assist with urgent prescription needs

Requirements:

  • Nurse Practitioner
  • Active and unrestricted WI license
  • Active and unrestricted DEA
  • Experience in palliative or hospice care strongly preferred
