SSM Health Dean Medical Group, a 450+ physician multi-specialty group, is actively recruiting a full-time (1.0 FTE) Palliative Care Advanced Practitioner to join our team at our Baraboo clinic.
Under the direction of the Division Chief, Palliative Medicine, the Advanced Practitioner - Palliative Care functions in an Advanced Practice role, the WI region Palliative Care Program providers co-manage patients with the referring specialists, they provide symptom management, discuss goals of care, promote quality of life, encourage coordination across multi-specialties, and facilitate complex patient and family decision-making. They will develop, implement, and evaluate care plans for adult patients with multiple or complex healthcare care needs in primary, acute, and long-term care settings, including the home.
The AP provides clinical expertise and mentorship to care team members throughout the continuum to enhance and inform palliative care delivery, serving as liaison to ensure continuity of care, communication and documentation.

Based in Baraboo, the Advanced Practitioner - Palliative Care, in collaboration with other Palliative Care Department providers, develops interdisciplinary programs, clinical practice guidelines, and educational materials to improve care for patients with palliative medicine needs throughout the SSM-WI Region.

The local hospital is St. Clare Hospital and Health Services, a 100-bed facility with 24 hour ER coverage. The hospital employs five full time emergency physicians. The hospital's primary service area includes the communities of Baraboo, Lake Delton, and Wisconsin Dells, with a population of 30,000. The surgical area at St. Clare Hospital has three operating rooms, one endoscopy room, and one Urology room. Block scheduling is done for each physician. There is a radiologist on site from 8:00 am - 5:00 pm Monday through Friday and on-call on weekends. MRI and CT services are provided at the hospital.
