Description

SSM Health - Illinois seeks a Bariatric Surgery Advanced Practitioner join their SSM Health - Good Samaritan Hospital and Employed Medical Group in Mt. Vernon, Illinois.

The Mid-Level Practitioner plans, coordinates and implements patient care specific to the age of the population served and within the clinical scope and specialty area of the Mid-Level Provider as designated by their respective certifying bodies and as delegated by their collaborating or supervising physician. The mid-level provider shall provide patient care according to the protocols agreed upon by the SSM Health - St. Mary's Hospital.

Responsibilities:

  • Assess client needs by - 1) interviewing clients and/or significant others to obtain history and conducting various diagnostic tests and procedures in order to provide qualified assessments and recommendations on the course of treatment, 2) performing initial and ongoing assessment of biopsychosocial, spiritual, cultural, and age appropriate needs, and 3) performing ongoing pharmacological management, including prescription use and review of medications.
  • Plans for patient care by - 1) making comprehensive clinical decisions based on advanced diagnostic skills and professional judgment, 2) review of results of diagnostic tests and procedures, 3) developing and implementing a treatment plan for the patient and/or family, and 4) providing a wide range of interventions required to effect change.
  • Interventions are implemented by - 1) prescribing and monitoring effectiveness of medications, 2) ordering diagnostic tests and procedures, 3) integrating assessment data into the treatment plan, 4) using medical, therapeutic, and corrective measures to treat illness and improve health status, and 5) providing patient education and teaching.
  • Evaluation of care is completed by - 1) collaboration with physician providing medical direction and consultation, 2) ongoing assessments to identify effectiveness of interventions, 3) modifying the treatment plan, and 4) collaborating with the multidisciplinary team as indicated.
  • Problem solving skills demonstrated by - 1) making competent advanced clinical decisions, 2) utilizing appropriate resources, 3) appropriate consultation with physician, and 4) appropriate involvement of management
  • Communicates effectively with customers by - 1) providing information for clients and families related to care, maintaining confidentiality, 2) providing information to the collaborating physician related to changes in the conditions of clients, and 3) documenting care given and response accurately.
  • Maintains a safe work environment - 1) adhering to the policies and procedures of OSHA, JCAHO, department and organizations, 2) ensuring appropriate operation of equipment, 3) following established infection control guidelines, and 4) reporting safety hazards.


Qualifications

Minimum Qualifications:

  • Successful completion of Master's Degree for Nurse Practitioner or Physician Assistant
  • NP/CNS - RN and Nationally Certified as NP or CNS in specialty area and Illinois License Advanced Practice Nurse -or- PA Certification as well as an active Illinois Physician Assistant license
  • Preferred for candidates to have at least two years of previous practice experience, but not required.
  • Excellent written and verbal communication skills. Able to assess the needs of the customer and effectively meet their needs.
